Transcript Requests

If you wish to order transcripts, download the Transcript Request Form then print the form, fill it out completely and mail it to:

Be sure to sign the form (the signature must be of that student requesting). Please note that there is a $15 charge per transcript requested.

Our Admissions and Records admits and registers students. Our records are maintained by the University in accordance with the California State Law. The University maintains student files for five years and transcripts for fifty years.
